to download the form
3) Obtain the downloaded form by clicking > Show In Folder
4) Select the downloaded form > right click > select “Open With” > select “Adobe Reader”
5) System Notification Warning > Click OK
6) IWK Registration Smart Form is ready for supplier/contractor to fill up and complete
